Explain how physician-led practices utilized state/local health departments or residency program partners to deliver evidence-based strategies and care coordination for preventing COVID-19 among minoritized and historically marginalized populations
Identify comprehensive strategies that center on equity to tailor care and interventions to meet the needs of minoritized and marginalized communities
Describe promising practices to address social needs barriers within communities at-risk for worsening COVID-19 by using existing tools and providing community referrals to patients.
